Steps to Wellness

An essential component of health is wellness. Personal wellness is a self-care approach to enhance our overall health and well-being. The Steps to Wellness is a personal tool kit that can help improve your health and well-being. The kit has several tools designed to work together to help you develop a plan for self-care, address a crisis should it arise, and advocate for yourself to receive treatments that work best for you. In addition to the Steps to Wellness kits, you may want to learn more about Minnesota 10 x 10, a program to increase the life expectancy of individuals lving with severe mental illnesses by 10 years in the next 10 years. This program outlines health screenings that should be done a regular basis to maintain overall health. Click on the link in the right side bar to learn more.

Self Advocacy Wallet Card
Self-advocacy is the act of speaking up for yourself and your needs. The self-advocacy wallet card gives you some helpful reminders on how to effectively prepare, document, and communicate your needs before you address an issue.
Medication Wallet Card
The medication wallet card allows you to organize your medications; write down your physician, pharmacy, and emergency contacts; and contains a list of common questions to ask your healthcare provider or pharmacist. Bring this card whenever you see a healthcare provider or receive emergency care.
Medication Form
This form is similar to the medication wallet card, but allows more space to document your medications if you have several types.
Daily Wellness Chart
Use this chart to track your daily steps toward wellness. It includes: a medication checklist, goals and reflections, exercise and activities, nutrition intake, and a sleep record. Keeping a pulse on your health and well-being is essential for both your body and mind.
My Crisis Plan
This worksheet is designed to help you identify early warning signs of crisis and plan ways to prevent a crisis from occuring.
Crisis Plan for your Support System
Give this worksheet to your supportive network of family and friends so that they know how to help you when you are nearing or in crisis.
Tips on Journaling and Feelings Guide
Journaling can be an effective way to reflect on your thoughts about your health and well-being. Creating a formalized process of collecting your thoughts can help you track your health.Use the Feelings Guide to find words that best describe how you are feeling.
Additional Tips
- Consult with your physician, therapist, and other health professionals. Having a collaborative approach can benefit your overall treatment plan.
- Share your goals and progress with a trusted family member or friend. Having someone in whom you can confide can give you much needed support.
- You can use all the wellness tools included in this kit or just use some of them. You are in control of your wellness!
